*Note New rules are in place at the Arizona Secretary of State's Office. You now must answer every single question, no blank spaces will be accepted. If you don't need to answer the question or it does not apply to you, please enter N/A into the field.

*Note New rules are in place at the Arizona Secretary of State's Office. You now must include a legible, "Clear" copy of your driver's license along with your bond, application and filing fee when submissions are being made to the Arizona Secretary of States Office.

Send the (Original) State of Arizona Notary Application for Notary Public Commission with your Original Notary Bond, A Legible copy of your Drivers License and a Check for $43.00 to Arizona Secretary of State
